Cambridge introduces comprehensive chronic illness support care model

Skilled nursing facility partners with Samaritan Palliative Care for new initiative

Special to The Sun: Samaritan Chief Medical Officer Stephen Goldfine (left), Cambridge Rehabilitation and Healthcare Center Director of Nursing Shanae Poole, nursing Assistant Director Sharon Jackson and Administrator Darren Fox celebrate the launch of the new Chronic Illness Support partnership Cambridge has established with Samaritan Palliative Care.

Enhancing its commitment to progressive skilled nursing care, Cambridge Rehabilitation & Healthcare Center in Moorestown has introduced a new, comprehensive Chronic Illness Support model, partnering with Samaritan Palliative Care to integrate palliative care, pastoral support and other services with its specialized clinical programming. Located at 255 E. Main Street, the 200-bed skilled nursing facility provides post-hospital care, short-term rehab and long-term residential care.

Cambridge developed its patient-centric Chronic Illness Support program based on a new platform recently introduced by Marquis Health Consulting Services, its administrative and consulting services provider.

“Cambridge’s specialized care initiative emphasizes a holistic focus on both physical and spiritual wellness for chronically ill patients – it is truly a next-level model of care supported by an excellent partnership with Samaritan,” said Marquis’ Kate Bauer, regional director of market development.

The palliative care component of Cambridge’s Chronic Illness Support program is administered by a trained team of doctors, nurses and specialists and focuses on providing emotional and spiritual support to help relieve symptoms and stress. The goal is to enhance the quality of life for patients and their loved ones. The overall initiative – which is specifically supporting Cambridge’s Cardiac Care and Neuro Recovery programs – also weaves in disease-specific education and health coaching, multi-specialist led and coordinated care, full time respiratory therapy, social services support and connections to community resources and life enrichment activities.

“We are proud to partner with Cambridge and their Chronic Illness Support program,” said Samaritan Chief Medical Officer Stephen Goldfine. “For more than 40 years, Samaritan has been committed to improving the quality of life for those we serve. Our palliative care offerings have allowed us to provide an extra layer of expert, compassionate care for patients and their families, and our collaboration with Cambridge introduces a patient-centric partnership aimed to reduce hospitalizations and improve health outcomes.”

Committed to serving as a healthcare resource for the community, Cambridge has established a resource library of physician-curated content, addressing many topics of interest for families of patients and the larger community. More information about the skilled nursing facility can be found at cambridgerehabhc.com.

The skilled nursing facility shares a 14-acre campus with Cambridge Enhanced Senior Living, an assisted living community featuring a memory care program appropriate for individuals in the earlier stages of dementia or Alzheimer’s. Both sit in the heart of picturesque downtown Moorestown, 12 miles east of Philadelphia.
