Gloucester County Mega Site is urging residents who lost their CDC cards to visit the New Jersey Immunization Information System (NJIIS).

If you lost your COVID-19 vaccination card after receiving your second dose, you can request your immunization record by visiting the New Jersey Immunization Information System (NJIIS).
