School district to have first-grade roundup May 29

On May 29, Evesham Township School District will be having its first-grade roundup.

This day is designated to registering any child who will be new to the district for the 2013–2014 school year.

Registrations will occur at any time throughout the school day at your neighborhood school, no appointments are necessary.

In order for a child to be eligible for first grade, he or she must be 6 years old on or before Sept. 30 of the school year.

In addition, the following requirements must be met before being permitted to enter the Evesham Township School District:
Parents must present proof of birth by either birth or baptismal certificate (not hospital certificate), it must be the birth certificate with the raised seal in addition to proof of the following immunizations, in accordance with the state Department of Health, signed by a physician: Diphtheria, Whooping Cough and Tetanus total of four doses, one dose of which must have been given on or after child’s fourth birthday or any combination of five doses.

Polio vaccine, three Sabin or two trivalent vaccines, or Salk polio shots plus booster (booster dose to be given on or after child’s fourth birthday) or any combination of four doses. The child must have two doses of measles, mumps and rubella vaccines, for any child born after Jan. 1, 1990. Both immunizations must be after the first birthday. Hepatitis B can be three doses of any vaccine combination containing Hepatitis B. Varicella can be one dose on or after the first birthday or proof of disease immunity.

Proof of immunizations must be available on registration day.

Parents must also provide two current proofs of residency, one of which should be a lease/agreement of sale or tax bill. The other document can include but is not limited to a utility bill, drivers’ license or a document with address printed on it.
A current physical examination within the past year is required of all first grade children.

Your family doctor or pediatrician should complete this.

At the time of registration, all documentation must be provided.

A copy of your documents will be made and the originals returned to you.
